A Vein of Galen malformation (VOGM) is an uncommon and life-threatening vascular anomaly; we present a case of a neonate who presented with congestive cardiac failure and pulmonary hypertension (PAH) secondary to VOGM. PAH was managed with sildenafil, and cardiac failure was brought under control. Embolization during the neonatal period has a high risk, so it was planned for a subsequent stage. The baby is currently stable, receiving anti-failure medication and taking direct breastfeeds.
